  1. Application Form
  2. Certificate of Member Representative Education of TURKDEX or other institutions determined by TURKDEX or Certificate of ISE or other exemption documents.
  3. A  notary approved declaration which states  that neither themselves nor the entities, in which they are unlimited liability partners have been adjudicated bankrupt
  4. Judicial records including the archived records, (it must be issued within 6 months)
  5. 2 passport-size photos of the candidate.
  6. A copy of the identification card approved by a notary,
  7. A copy of the certificate of graduation and copies of the translation of the diploma and equivalence certificate thereof issued by the Board of Higher Education (YÖK), all must be notary approved.
